Promoting Access to Health (PATH) for Women (Los Angeles, CA)

Goal: The goal of this program is to extend lifesaving prevention programs and screening services across a variety of cultures by giving communities the materials and plans to implement an effective intervention while allowing them to execute these plans in a manner and at a pace that resonates with their own culture and community.

Reconciliation Services Depression and Trauma Therapy Services (Kansas City/Jackson/Missouri)

Goal: The mission of Reconciliation Services is to cultivate a community seeking reconciliation to transform Troost from a dividing line into a gathering place, revealing the strength of all. Our mental health services are at the heart of how we strive to help clients see their own strength and work towards being reconciled to their own health and well-being.

Impact: Over the past three years, our therapy program participants reported a 71 percent decrease for depression and 79 percent decrease for PTSD.

Red Rock Ridge and Valley Trail System (Jefferson County, AL)

Goal: The Red Rock Ridge & Valley Trail System will be a complete network of bike lanes, sidewalks, and trails connecting communities across Jefferson County.

Impact: The master plan for over 200 miles of shared-use greenways and trails and over 600 miles of street-based bicycle and pedestrian pathways will improve alternative transportation and recreational opportunities.

Reducing Health Disparities: Diversity and Cultural and Linguistic Competence in Contra Costa (Contra Costa County, CA)

Goal: The goal of this program is to eliminate health disparities among the diverse patient population in Contra Costa County by improving access to services for people who are not comfortable speaking English, and by increasing the cultural and linguistic competence of staff in order to have a workforce capable of working effectively with diverse patients, clients, customers and communities.

Responding in Peaceful and Positive Ways (RiPP)

Goal: The goal of this program is to prevent violence among middle school students.

Impact: Rates of violence-related violations among 6th graders were 2.2 times lower in classrooms using RiPP than in non-participating classrooms. In-school suspension rates among 6th graders were also 5.0 times higher in the non-participating classrooms.

Safe Passages, Early Childhood: The Oakland Early Childhood Initiative (Alameda County)

Goal: The goal of Safe Passages' Early Childhood Initiative is to reduce the impact of violence on young children and to encourage the development of positive social skills and healthy family environments in order to prevent future violence.

Safer Choices (California, Texas)

Goal: The goal of the Safer Choices program is to reduce the number of students engaging in unprotected sexual intercourse.

Impact: The program reduced the frequency of intercourse without a condom, reduced the number of sexual partners with whom students had intercourse without a condom, and increased use of condoms and other protection against pregnancy at last intercourse.

Salud Para Su Corazón (Health for Your Heart) (Washington D.C.)

Goal: The goal of Salud Para Su Corazón is to raise awareness and knowledge of heart disease risk factors and prevention among Latino populations.

SCORE Concussion Education and Baseline Testing Program (Greater Washington, DC)

Goal: The goal of the community education, training and baseline testing component of the SCORE Concussion Program is to improve community understanding of concussions and response post-injury.

Impact: In 2012, The SCORE Concussion Education and Baseline Testing Program provided baseline testing and student athlete education to 1,522 children, lead 32 parent and coach education sessions, and conducted workshop training in more than 30 schools.

SmartWay Transport Partnership (USA)

Goal: The goal of this partnership is to improve the environmental performance and fuel efficiency of the US freight sector (truck & rail) through the use of a voluntary market incentive system and to encourage retailers/end users to choose trucking and/or rail companies that are environmental leaders in their respective industry segments.

An Effective Practice meets the following HCI criteria:

The program description includes at a minimum: the sponsoring organization, program goals, program implementation steps, and outcomes that have demonstrated program success and/or promise in achieving the program goal in one or more localities.
The results from an evaluation of the program include quantitative measures of improvement in outcome of interest (i.e. increase in voter registration, not just delivery of voter registration drive) and/ or the outcome measure is increased or improved compared to the baseline measurement or the control group but the difference is not statistically significant.

A Good Idea meets the following HCI criteria:

The program description includes: the sponsoring organization, program goals, program funding source, program implementation steps and outcomes.
The program evaluation is limited to descriptive measure(s) of success/accomplishment (i.e., program participation rates, number of services/education sessions/radio messages provided). Note: oftentimes, the program has been newly implemented and a program evaluation has not yet been conducted. Programs that have not yet been evaluated, but which show promise in improving health or quality of life, are classified as Good Ideas until an evaluation is conducted.

An Evidence-Based Practice meets the following HCI criteria:

The program description includes at a minimum: the sponsoring organization, program goals, program implementation steps, and outcomes that have demonstrated program success in achieving the program goal in one or more localities.
The results from an evaluation of the program include quantitative measures showing improvement in the outcome(s) of interest after the implementation of the program (i.e. increase in smoking cessation, not just the delivery of a smoking cessation program). The outcome measure(s) is/are compared at relevant time periods before and after the intervention or program implementation. Alternatively, the evaluation study compares the outcome(s) between an intervention group and an appropriate control group.
The study is of peer-review quality and presents numbers in a scientific manner; measurements of precision and reliability are included (e.g. confidence intervals, standard errors), results from statistical tests show a significant difference or change in the outcome measure(s), and relevant point estimates and p-values are presented. Note: if the results from an evaluation of a program are presented in a scientific manner and the outcome measure is improved compared to the baseline measurement or the control group but the difference is not statistically significant, the practice is classified as effective and not evidence-based.
