The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

The goals of the MCM are to provide a comprehensive and centralized concussion care program to 1) increase concussion awareness and identification through education and training; 2) facilitate the return to play decision with effective medical treatment, which includes baseline neurocognitive testing; and 3) implement a standardized concussion care protocol and concussion injury surveillance system to assist in the prevention of concussions, improve player safety, and limit school liability.
From the pilot evaluation of the model it was determined that the MCM model or a similarly designed one is effective in increasing the number of concussions identified, reported, and also treated at a clinic.

The objective of this article is to describe Project Restoration's approach to cross-sector collaboration with a shared population of high utilizing community members, describe the process and outcomes of the collaboration, provide strategies to mitigate challenges that arise during collaboration and offer insights to inform similar initiatives in communities nationwide.
Hospitals across the region are assessing ways to establish a centralized infrastructure to collaboratively address the complexities of individual cases, as well as a regional council to explore process improvements to enhance the lives of vulnerable populations.

The goal of the Diabetes-Based Science Education for Tribal Schools (DETS) curriculum is to slow or reverse the rising rate of type 2 diabetes in American Indian/Alaskan Native (AI/AN) youth through a pedagogy based in a combination of a science-based diabetes/health education curriculum and culturally relevant contexts.
Overall, the DETS curriculum shows that collaboratively-developed curriculums and education courses can have an effective impact across grade levels with students having significant knowledge gains, and can also serve as a supplement for other science and social science curriculums in schools.

As a leader in Kansas City's emergency food network, ECS is committed to providing access to healthy food for the community's food-insecure households. Its mission is to engage the Episcopal and broader communities in feeding the hungry and empowering the poor to move beyond the barriers of poverty with dignity - in short, feeding the hungry & changing lives. ECS is best known for the Kansas City Community Kitchen (KCCK) in the heart of the urban food desert. ECS also works to provide meaningful training experience through the Culinary Cornerstones Training Program, a 30-week immersive program preparing individuals for careers in the culinary world.
Since implementing the new service model, there has been a 10% average increase in the number of daily meals served at the Kansas City Community Kitchen. There has also been a large increase of volunteers, with an increasing number of recurring volunteers.
Direct Mailing of Fecal Occult Blood Tests to Increase Colorectal Cancer Screening (Wright County, MN)

The goal of this program was to increase colorectal cancer screening using direct mailings of fecal occult blood test (FOBT) kits for noninvasive colon cancer screening.
Direct mailing of FOBT kits resulted in an increase in self-reported colon cancer screening adherence.

The Edible Trees Program seeks to increase local access to fruiting trees and provide equitable access to healthy foods.
The Edible Trees Program provides communities access to fruit and nut trees. The program has funded 19 projects resulting in 1,320 trees & shrubs planted.

By purchasing shares in the Faith Community United Credit Union (FCUCU), individuals gain access to services and learn the difference between using what belongs to someone else and owning their own institution.

Making Connections for Mental Health and Wellbeing Among Men and Boys is a national initiative to transform community conditions that influence mental wellbeing. The Prevention Institute works with 13 communities across the U.S. to shift policies, practices, and norms to create greater opportunities for health and resilience, with particular focus on veterans and men and boys of color.

The goal of EnhanceFitness is to encourage older adults to engage in regular physical activity to improve their health and well-being.
EnhanceFitness participants reported a 13% improvement in social function, a 52% improvement in depression, and a 35% improvement in physical functioning. Additionally, participants' healthcare costs were 21% less than those of non-participants after one year.
